Thread: ATOS ESA medical assessment appointment, what do I take with me?

ATOS ESA medical assessment appointment, what do I take with me?

Am I entitled to contact my doctor to request a supporting letter? I have at least 4 appointment letters with the hospital for various procedures, I am precribed morphine for pain control.

ATOS have a terrible reputation in the press and I think it is thoroughly deserved but I need to effectively put my case across or I will have to go through the appeals process.

    Any tips would be appreciated.
    Thanks

    HI karljt
Apart from yourself you will need to take
    1) your appointment letter , proof of id passport or driving licence , and something to prove your address i.e utililty bill or bank statement
    2) any medical evidence u have like letters from your doctor or appointment letters and any medication your taking .
    3) your bank details if you are claiming back your travel expenses .

yes your entititled to contact your doctor for a medical letter but be warned the doctor will charge a fee for writing you a letter

Hope that helps all the best with assessement

You can request a letter from your doctor, but the doctor is entitled to charge or refuse entirely.

You might find copies of hospital letters (which might be sent to you routinely) or an extract from your medical notes provide all the information you need.
