    Question about DLA form? Who fills page 4?

    Hello all,

    I was just wondering if anyone knew the answer to this question, on page 4 of the DLA form does my mother put her name and address in the details on that page as she is filling out the form for me? She is not signing the form for me, I can sign my signature. Hope someone can help.

    Indeed, if mother has filled out the form for you, then theres no harm in her completing page 4, telling the DLA people this. As you say, you can sign it yourself so i assume that you dont need someone to act on your behalf, it's just that she filled it out for you. No worries.

    I assume this is a DLA 'review' claim due to DLA's abolition.
