New application form for 'New Style' ESA

As of today there is a new form to use to make a claim for New Style CB-ESA.

It's called a NSESAF1

You can download it from here:

There is one to fill in on screen, or one to print and use a pen.

You cannot post it. - You will need to make an appointment at the Jobcentre and take the completed form with you to make a claim.
