Your GP can't "refuse" you a social worker. Why do you wish a social worker? If you are having difficulty with daily life and feel you need extra help, you can self refer, ie ring social services, explain the situation and they will come and do an assessment.
They will ask IN DETAIL, how your back pain affects you ability to cook, wash, dress, walk etc.
All this takes time (the only thing you can do is keep chasing them) and may not result in any help BUT you can do that.

CAB are probly the best to help as they will have all round knowledge of other organisations. If you can get the specialist from 3 yrs ago to write a letter, even better.
