    address for PIP forms

    Hi my friends son in law has got to have his kidneys removed and to go on dialasis( sp)( Very complicated to explain)and I have suggested to her to tell her son in law to try to claim DLA/PIP but I don't know the address to ask for the forms to be sent and I wonder if any of you lovely people know the address.

    You need to phone to start a PIP claim.
    They will then send out the required forms.

    NOTE: These PIP application forms have a personal barcode on them that refers to each application, this is why you have to phone and just can't download one or pick one up from an office.

    There is a qualifying period (I think 9 months) that you have to have the condition before you can claim PIP.
