Please clarify - you've never received DLA? This is your first PIP application?
Only charity I can think of is 'back to the Cancer team' and asking them. As they would offer their services voluntarily - so no cost to you, but you wish to pay to drive and pay to park.
It is funded from charity. In fact about 10 years ago my brother had an accident and was sent to another hospital by 'volunteer' transport - no charges were made.
In the case of my mother - again...

Curious - what kind of 'card'? Prior to receiving PIP I received DLA - I've never had a card saying I'm registered as disabled? Who, where did the card come from?

My mother's specialist was 50 miles away - e.g. round trip 100 miles - the Cancer Department have 'volunteers' who pick up and drop off and collect again. Have you tried them?
ESA and PIP are separate
ESA - is ability to work split into WRAG and Support.
PIP - is day to day care
Presume from the brief 'statement' you are in ESA WRAG and want to transfer to Support...

Ok just another idea, Red Cross and similar charitable organisations do loan wheelchairs - you could try them and 'borrow' with a donation. Just a thought.
If she can't eat - doh - she'll starve to death - in my opinion it's manslaughter or murder - and it was the NHS who recommended the darn surgery! :( After the surgery they advised the cancer tumour...
