I would greatly appreciate if you could fill in this survey for me - as it will help in a research project my boyfriend is doing in the hope of setting up a disability tennis programme at his tennis club. Question 5:
Are you registered as having a disability?
is not really a valid question as you haven't been able to register with a disability since the 1970's.
A more suitable question would be to ask if you receive DLA HRM or the Enhanced Mobility PIP?

Also spelling mistakes in question 12's answers don't help.
